**Health Promotions Consultant, Sr. /Community Health Nurse/Public Health Nurse -Denver, CO - P26606**
Summary: The **Health Promotions Consultant, Sr. /Community Health Nurse/Pu** **blic Health Nurse** is responsible for ensuring gaps in care closure, health education requirements, or other programs required by contracts.
Primary duties may include, but are not limited to:
+ Defines the health outcomes environment through development of key stakeholder criteria, identification of desirable community partners, availability of local programs and opportunities.
+ Provides/promotes provider training to assure compliance with government mandated health education programs and the health education referral system.
+ Establishes positive relationships with community based organizations and other key partners to gain understanding of the cultural and linguistic needs of members.
+ Recommends health education programs based on analysis of State/Regional/Local health statistics, disease prevalence, census data, and health care service usage.
+ Maps community based assets and collaborates with key stakeholders to leverage available resources for development of new and accessibility to existing health education/promotion programs.
+ Develops and implements strategies that promote local health education resources and address identified member health education needs
+ Requires a BSN; 4 years of clinical experience in health care /managed care and with data analysis and interpretation; or any combination of education and experience, which would provide an equivalent background.
+ Current unrestricted RN license in applicable state(s)
+ Must have ability to comprehend medical policy and criteria to clearly articulate health information.
+ Local Travel required.
+ Medicare and Medicaid knowledge- highly preferred
+ Able to coordinate outreach to members in the community to attend local events and maintain a partnership; member facing; cultural alignment
+ Network/Identify with existing vendors, Expos to participate and align Anthem with scheduled events
+ Must be able to analyze data to better understand the population care gaps to get members to the health events/fairs
+ Experience as a community health worker explaining wellness prevention to the population
+ 3 years of HEDIS experience highly preferred
+ Background in Community/Public Health experience highly preferred
