Responsibilities
Perform concurrent and retrospective chart reviews for core measure abstraction. Assist with departmental activities as needed, including but not limited to, case investigation, ACA/RCA activities, team member survey readiness and participation in on-site survey process. Assist with implementation of action plans established by system RPI team and monitor completion of action plans established by RCA team. Assist with facilitation of peer review and collaborate with leaders on departmental PI projects.
Assists with planning, coordination, implementation and evaluation of the organization-wife performance improvement plan and program in accordance with the Joint Commission standards and CMS and NJ Department of Health mandates. Utilizes the theories, methods and tools of robust process improvement including apparent cause analysis and root cause analysis.
Assist with performance improvement and quality initiatives.
Qualifications
* 3-5 years' experience in nursing, social work, quality improvement, team member education or other appropriate clinical area required.
* BSN or LSW required
* Quality improvement or regulatory experience preferred
* Active NJ License in area of clinical practice.
