The Social Worker/ Supervisor will have the ability to meet monthly client service unit goals. Keeps records and prepares daily, weekly, monthly, and annual statistical and administrative reports. Uploads mandatory reporting data onto the DFTA website and retains all supporting paperwork.Counseling: Provides individual and group counseling as needed.Education: Provides Life Skills and Behavioral Education.Conducts Initial Interviews: Conducts Initial Interviews of members to determine their Social Service needs if any. Documents Social History, performs Social Services assessment, develops Social Services Plan of Care if necessary, writes Social Services progress notes.Problem Solving and Complaint Resolution: Receives all grievances. Finds solutions to individual problems relating to immediate needs and refers members to appropriate community agencies and services. Determines when a member's grievances need to be brought to the attention of the other staff members. Assists with ongoing problems related to the congregant meal program and activities.Crisis Intervention: Steps in when conflict or life cycle emergencies occur at the center. Provides services to temporarily homebound/hospitalized members and makes sure they are receiving help from social services or family members.Member Contact: Making routine rounds to keep open lines of communication. Stimulating social contact and interaction by encouraging engagement in center activities. Identifying Social and Emotional needs of members and making program recommendations based on those needs.Establishing Linkages: Establishing links with outside agencies for social services needed by members but not available within HANAC INC. Contacting and utilizing community resources on the resident's behalf and serving as a link between the resident and these resource systems when necessary. Promotion of the center by speaking to community groups about available social services at the facility.Financial and Benefits Assistance Counseling: To act as a source of information regarding available governmental or private funding sources and to assist in the completion of the associated paperwork.
Facilitating Groups: Leads Bereavement Groups, Caregiver Support Groups, and supervises Discussion Groups. Assists in the development or implementation of programs for the elderly that meet the federally mandated requirements.
Additional Requirements:Work to ensure the center environment is free from verbal, sexual, physical, or mental abuse, corporal punishment, and involuntary seclusion.Work to ensure the center's environment promotes maintenance or enhancement of each resident's quality of life.Advocating for and protecting member's rights.
Qualifications:LMSW or Mental Health Counseling Degree2 - 5 years working directly with clientsCounseling ExperienceAbility to meet goals in a timely mannerCoordinator of Social ServicesAbility to work in cooperation with HANAC Extended Care ServicesCrisis Intervention SkillsAbility to document and report all social service contract and make appropriate referralsAbility to understand the problems of older adults, minority and disadvantaged groupsAbility to produce documents written in the English language using proper sentence, punctuation, grammar, and spelling.Willingness to stay current in the field by attending seminars, training and relevant community events
Required work Schedule:
