Responsibilities

This position organizes and leads activities, including required screenings from DHS, tracks data, and ensures contract requirements are met. Coordinates with other community agencies providing services to clients. The Case Manager-Skills Trainer interviews and evaluates clients, and helps formulate and carry out individual treatment plans. Assists clients with accessing local, state and federally-funded services. Drives agency vehicle and transports clients as required. Submits timely documentation of services provided using Electronic Health Record.

Qualifications

Bachelors (BA/BS) degree in a human services field, OR three years of experience providing services in a similar setting with individuals who have emotional and/or mental disabilities, OR a combination of education and experience that provides the desired skills, knowledge, and abilities required to perform the job. Related experience preferred.

Candidate must be able to work independently and flexibly, under general supervision.

Computer proficiency is required. Must possess strong organizational skills and the ability to read and understand a medical record. Training is provided on the Electronic Health Record System. The candidate must pass state-required background and DMV checks, possess a valid OR driver's license, and have an excellent driving record.
