The Case Manager for At-Risk Youth will serve at-risk youth, with and without disabilities, and their families. The successful candidate will be expected to:

  • Work with youth who are at-risk;
  • Complete intake assessments;
  • Provide information and referral services;
  • Our staff are creative, self-starters who build relationships and juggle multiple priorities: independent, yet able to work on a team of dedicated individuals.

  • Conduct a youth risk assessment to determine their potential for crisis and involvement with the juvenile/criminal justice system; and
  • Maintain regular contact with participants and their family members to monitor progress.
  • Rather than being expected to keep quiet and surrender to barriers within the human service system, our staff are urged to rock the boat, start debate, and to make positive change. To achieve that end, employees are expected to advocate, regardless of position.

    Who may be considered:

    Knowledge of ADA, IDEA, 504, Social Security, ACCESS-VR, CBVH and the juvenile/criminal justice system is preferred.

    One-to-two years of experience working with individuals who have disabilities is required.

    An Associates Degree in Human Services or relevant experience is required.
